Seven-year-old Antonia lives in Haiti with her parents and younger brother. “Her parents both work in the marketplace to support the family,” shares our medical partner, Haiti Cardiac Alliance (HCA).

Antonia is in second grade and loves going to school, but she’s had to miss class frequently this year due to symptoms of her heart defect. HCA explains, “Antonia was born with a heart defect called partial atrioventricular canal defect, in which holes exist between the upper and lower chambers of the heart, allowing blood to pass freely through all four chambers. This leads to heart failure and deprives the body of oxygen, leaving her sickly and weak.”

Heart surgery can correct Antonia’s defect, allowing blood to flow normally through her heart. Gift of Life International has raised $5,000 to pay for part of her surgery. For $1,500, we can fund the rest of Antonia’s life-changing surgery, which includes preparation and overseas transportation costs.

“We are excited she can have this surgery so she can get back to her education,” her mother shares.

Antonia received life-saving heart surgery.

Antonia’s surgery was a success! “During surgery, the hole in Antonia’s heart was closed with a patch. She should be able to live a normal life with no further symptoms from this condition,” reports her doctor at Haiti Cardiac Alliance.

In her post-operative interview, Antonia was excited to return home. “I am very happy that the surgery is over and I get to go home and see all of my friends,” she shared.

What kinds of symptoms do patients experience before receiving treatment?

When a hole exists in the heart, a physician can hear a buzzing noise, or murmur, in the child's chest as blood passes through the hole at high velocity. The child's parents might notice that their son or daughter cannot keep up with other children in daily activities. In severe cases, the lack of oxygen in the bloodstream can lead to dramatic symptoms, such as blue lips and tongue, clubbed fingers and toes, and heart failure. The patients treated by Haiti Cardiac Alliance tend to fall into two categories. They are either born with some type of hole or defect in the heart, or they develop valve disease as a result of an untreated strep throat infection (rheumatic fever). Patients with rheumatic valve disease experience swelling of the abdomen and extremities, as the heart tries to circulate blood through the body despite the valve's dysfunction.

What is the impact on patients’ lives of living with these conditions?

Virtually all of the conditions treated at Haiti Cardiac Alliance will eventually lead to death without surgery, the majority of them within one to two years. In the meantime, patients experience heart failure as their hearts struggle to compensate for the presence of leaks or other defects. In most conditions, the heart becomes fatigued, limiting the child's ability to be active, go to school, and participate in daily life.

What cultural or regional factors affect the treatment of these conditions?

Families in Haiti often have complex cultural mechanisms for understanding cardiac illnesses and their causes, sometimes involving voudou or other religious belief systems. Nevertheless, the overwhelming majority of Haitian families in our medical partner's program also engage with the medical explanations and treatment of these conditions. Parents are willing and cooperative participants in their child's treatment.

What does the treatment process look like?

The patient is first referred to our medical partner, Haiti Cardiac Alliance (HCA), by a pediatrician or another medical practitioner who detects symptoms that might be cardiac in nature. HCA staff then perform an echocardiogram to diagnose the cardiac condition. If surgery is required, the staff decides whether the child can be treated in-country or needs to be flown elsewhere to access care. If the child can be treated in-country, he or she is scheduled for an upcoming surgical mission. In the meantime, HCA provides periodic cardiac checkups. Typically, the child spends 4-5 days in or near the hospital prior to surgery for testing and examinations. After surgery, he or she spends several more days as an inpatient prior to being discharged. HCA provides regular cardiac checkups for at least five years postoperatively before the final discharge from their program.

What is the impact of this treatment on the patient’s life?

These treatments are almost always life-saving in nature. These cardiac conditions are not survivable over the long-term without surgery. Within weeks after surgery, the patient should notice a difference in energy level. Many patients also undergo a growth spurt and/or gain significant weight after a surgery.

What potential side effects or risks come with this treatment?

The risk of death during or shortly after an open-heart surgical procedure is about 3%. Other risks, though rare, include stroke and post-operative infection. In a small percentage of cases, the material used to patch the hole "blows," and a follow-up surgery is necessary to re-patch the defect.

How accessible is treatment in the area? What is the typical journey like for a patient to receive care?

Patients come to Haiti Cardiac Alliance (HCA) from the entirety of Haiti. This can involve three days of travel in buses, pickup trucks, or even on horseback. There is no cardiac surgery of any kind available in Haiti outside of the HCA treatment network.

What are the alternatives to this treatment?

In general, patients are treated with medications to prevent heart failure until they are ready to travel. Patients may also seek care from traditional healers, who may use liquids and powders derived from local plants and roots.

Mariam is a newborn baby in a family of two. Mariam's parents are farmers who work diligently on their farm to provide food and sell surplus harvests for additional income. Miriam was born with clubfoot on both feet. Her feet are twisted inward and downward. This causes difficulty walking and even wearing shoes. Fortunately, Mariam's family traveled to visit our medical partner's care center, Kafika House. There, the team will begin clubfoot treatment on April 5th. Our medical partner, African Mission Healthcare, is requesting $935 to fund Mariam's clubfoot repair. After treatment, her mobility will improve, and she will be able to wear shoes properly. Mariam’s mother says, “Please help my daughter. I hope she gets treatment and grows up to enjoy a life free from disability.”

Briven is a 2-year-old boy from Uganda. His parents are farmers who grow food for the family. He has two older brothers who are in school. Briven's father hopes his son will be an engineer in the future. Briven was born with hydrocele or a swelling in a sensitive area of the body. The doctors recommended surgery for him once he reached eight months. However, his parents couldn’t raise the funds needed. Recently, Briven has been in pain, unable to walk or play. Fortunately, on March 12, he will undergo hydrocele repair surgery. Our medical partner, African Mission Healthcare Foundation, is requesting $139 to fund Briven's surgery. Once completed, this procedure will hopefully allow him to live more comfortably and confidently. Briven's parents shared, “The only hope we have for a complete recovery of our child is by surgery, yet we have no money. We really need support.”

Reaksa is an 11-year-old fifth grader from Cambodia. She is the oldest of four children. Reaska's mother stays home to look after the children, while her father works for a local construction company. At school, Reaska enjoys learning Math and Khmer history. Reaksa wants to become a nurse when she's older. In her free time, she plays with her siblings and the other kids in the village. She also loves to draw. Reaksa has been experiencing frequent sore throats, difficulty swallowing, snoring and coughing at night, weight loss, and recurring fevers. These frequent illnesses have caused her to miss school. Reaksa was recently diagnosed with enlarged tonsils and adenoids, which, if not treated, will cause her symptoms to persist and possibly intensify over time. Our medical partner, Children's Surgical Centre, is requesting $265 to fund a tonsillectomy and adenoidectomy for Reaksa, which is scheduled to take place on March 18th. Surgeons will remove her tonsils and adenoids, hopefully relieving Reaksa of her symptoms and helping her live much more comfortably. After undergoing surgery, Reaksa hopes to enjoy meals without discomfort and attend school regularly.
