Souhaimy is a 20-year-old student from Cambodia. He is from a rural area and has four brothers. His father is a construction worker, and his mom sells fried noodles from a street cart. In his free time, he and his friends play soccer, go on social media, and do homework under a tree near their house.

When he was a child, Souhaimy had an ear infection. This infection caused a cholesteatoma, or abnormal skin growth, to develop in the middle ear behind the ear drum. For this reason, Souhaimy experiences hearing loss, tinnitus, and ear discharge. It is difficult for him to hear and he is regularly taking various medications, but to no avail.

Souhaimy traveled to our medical partner’s care center to receive treatment. On February 9th, he will undergo a mastoidectomy procedure in his left ear. During this procedure, ENT surgeons will remove the cholesteatoma. Our medical partner, Children’s Surgical Centre, is requesting $926 to fund this procedure. This covers medications, supplies, and inpatient care.

He said, “I hope my ear is better after surgery. I am embarrassed about the discharge and I can’t remember my ear ever feeling normal.”

Souhaimy underwent ear surgery to relieve him of pain and he is now headed back to school.

Souhaimy traveled three hours to our medical partner in the capital city and had successful surgery to remove the cholesteatoma, which was critical to avoid permanent hearing loss. He will keep his ear wound dry, and return in two weeks to remove the sutures. After his ear heals, his frequent ear infections should disappear and his hearing should improve. His family is happy they no longer will have to pay for medication that proved ineffective. Souhaimy looks forward to improved hearing without the disruption of ear infections, and now has a positive outlook because he can continue his schooling to accomplish his goals.

Souhaimy’s mother shared, “We are happy and thankful to the staff for the success of the operation. Souhaimy can go back to school and we will be proud. CSC is a good hospital to have surgery.”

What kinds of symptoms do patients experience before receiving treatment?

A mastoidectomy is a surgical procedure that removes diseased mastoid air cells. A patient who needs a mastoidectomy will experience hearing loss, chronic ear infections, and possibly cholesteatoma—an abnormal skin growth in the middle ear. Cholesteatomas cause hearing loss and ear discharge. The cholesteatoma will erode bones in the middle ear and can eventually expose the brain and cause death in complicated, untreated cases.

What is the impact on patients’ lives of living with these conditions?

Patients live with hearing loss and chronic ear infections.

What cultural or regional factors affect the treatment of these conditions?

Treated incorrectly or left untreated, an infected mastoid bone can cause hearing loss, persistent ear discharge, meningitis, temporary dizziness, and temporary loss of taste. Due to poor hygiene and limited education in rural Cambodia, patients are likely to experience complications and receive the incorrect treatment.

What does the treatment process look like?

A mastoidectomy is performed with the patient asleep under general anesthesia. Surgeons will perform one of several different types of mastoidectomy, depending on the amount of infection present. These include: • Simple (or closed) mastoidectomy: The operation is performed through the ear or through an incision behind the ear. The surgeon opens the mastoid bone and removes the infected air cells. The eardrum is incised to drain the middle ear. Topical antibiotics are placed in the ear. • Radical mastoidectomy: This procedure removes the most bone and is usually performed for extensive spread of a cholesteatoma. The eardrum and middle ear structures may be completely removed. Usually the stapes, the "stirrup"-shaped bone, is spared to preserve some hearing. • Modified radical mastoidectomy: In this procedure, some middle ear bones are left in place, and the eardrum is rebuilt by tympanoplasty. After surgery, the wound is stitched up around a drainage tube, and a dressing is applied.

This treatment will relieve chronic ear infections, hearing loss, and other symptoms caused by the infected mastoid bone.

This treatment is highly effective, but it poses risks if performed by an inexperienced surgeon. The operation is near the facial nerve and the brain, so surgeons must be careful when operating. At Children's Surgical Centre, ENT surgeons only operate on cases about which they feel confident.

How accessible is treatment in the area? What is the typical journey like for a patient to receive care?

Care for this condition is not easily accessible in Phnom Penh. Only one other hospital performs ENT surgery, but care at that hospital is expensive. The ENT surgeons at our medical partner have a proven record of successful cases.

What are the alternatives to this treatment?

Once the infection stops responding to antibiotics, surgery is the only option.

SreyPov is a skilled 43-year-old weaver specializing in making silk and cotton skirts. She lives with her husband, who works in construction, and her three children in Phnom Penh province. Her 19-year-old daughter works in a restaurant, her 15-year-old daughter is in school, and her 9-year-old son is also in school. SreyPov diligently weaves traditional skirts at home, contributing to the local businesses, and takes pride in maintaining her household's cleanliness. In September 2023, SreyPov slipped on a wet sidewalk, stretched out her right arm, and dislocated her elbow when she fell. Given her family's financial constraints, SreyPov sought the assistance of a Khmer traditional healer. However, despite her efforts, her arm remains swollen, causing persistent pain. Fortunately, surgeons at our medical partner, Children's Surgical Centre, can help. On January 4th, SreyPov will undergo a fracture repair procedure, which will cost $483. This surgery will allow her to resume her traditional weaving and alleviate her pain. SreyPov said: "I hope after surgery I can make cloth again and will not be in pain any longer."

Peter is a farmer from Rukungiri district in southwestern Uganda. He is a father to four children who are all in school. Peter and his wife grow food crops for their family's consumption and sell the surplus to generate income. For three years, Peter has had a left inguinal hernia, causing swelling and pain. Doctors recommend that he undergo surgery and, fortunately, on April 3rd, Peter will undergo hernia repair surgery at our medical partner's care center. Our medical partner, African Mission Healthcare, is requesting $230 to fund Peter's surgery. Once completed, this procedure will hopefully allow him to live more comfortably and confidently. Peter says, “I hope for a complete recovery and will be able to resume farming in good health to sustain my family.

Gerson is a four-month-old baby. He is the second-born child in a family of two children. Gerson's mother stays at home while his father engages in casual day jobs to earn a living. Gerson was born with clubfoot on both his legs. His feet are twisted inward and downward, which requires treatment at an early stage. This will make it difficult for Gerson to crawl, walk, and wear shoes. Fortunately, Gerson's family traveled to visit our medical partner's care center, Kafika House. There, the team will begin clubfoot treatment on April 5th. Our medical partner, African Mission Healthcare, is requesting $935 to fund Gerson's clubfoot repair. After treatment, he will be able to wear shoes and will reduce stress on his feet when he starts walking. Gerson’s mother Says: “Please help my son. I hope he finishes his treatment and his feet get better to grow up and be successful.”
