Fyness is a 45-year-old mother of three from Malawi. After the loss of her husband, Fyness became the sole provider for her family. She works hard to support and raise her children.

After developing troubling symptoms, Fyness went to the hospital, where she was diagnosed with cervical cancer. Physicians have scheduled her for a hysterectomy, a surgical operation to remove all or part of the uterus. The surgery is scheduled for February 7. Our medical partner, World Altering Medicine, has requested $643 to fund the procedure.

This surgery should improve Fyness’s current symptoms and protect her from further spreading of the cancer.

Fyness underwent a hysterectomy procedure.

Fyness’s operation was a complete success with no complications. Now she is feeling much better. Fyness will now be able to care for her family and her land without the pain and discomfort she was experiencing prior to her surgery.

What kinds of symptoms do patients experience before receiving treatment?

Patients requiring hysterectomies at our medical partner's care center generally have one of three conditions: uterine fibroids, cervical cancer, or chronic bleeding. Women with uterine fibroids may experience chronic pelvic pain. Women with cervical cancer are often symptom-free and are only diagnosed upon screening. Finally, women with chronic bleeding experience atypical and excessive vaginal bleeding.

What is the impact on patients’ lives of living with these conditions?

These three conditions may impact the patient's life in different ways. Women with pelvic pain and bleeding may be unable to work or be active. Women who experience bleeding may feel weak, due to the loss of blood and inaccessibility of blood products in Malawi. Women with cervical cancer are at risk of cancer metastasis.

What cultural or regional factors affect the treatment of these conditions?

Cervical cancer is the top killer of women in Malawi, which has the highest rate of this type of cancer in the world. There are several factors that lead to these high rates, including early sexual initiation, high HIV prevalence, and lack of screening and treatment. Although health facilities nationwide are supposed to provide screenings, many do not. Therefore, women are never screened, and the cancer can progress to an untreatable stage. In addition, women with cervical cancer often experience no symptoms and may not go in for a screening. Women with vaginal bleeding may experience stigmatization. In Malawi, menstruation can be a challenge. It is difficult to keep fabric clean, as there is often little access to sanitary products.

What does the treatment process look like?

A hysterectomy is a surgical operation to remove all or part of the uterus. Surgeons perform radical hysterectomies on women with cervical cancer. This is an aggressive surgical approach to combat cancer. Women with fibroids or bleeding may receive alternative treatments before a hysterectomy.

What is the impact of this treatment on the patient’s life?

For patients with cervical cancer, a hysterectomy is a lifesaving operation. Without treatment, the cancer will continue to spread and ultimately become untreatable. Chemotherapy has not been proven effective in treating cervical cancer, so a hysterectomy is the best option for women. Women with fibroids or bleeding will be able to return to their daily lives without chronic pain and bleeding.

What potential side effects or risks come with this treatment?

There are risks associated with any surgical procedure. In this case, there is a risk of damage to the ureters and the tubes that connect the kidney to the bladder, as they are near the reproductive system.

How accessible is treatment in the area? What is the typical journey like for a patient to receive care?

There is one public hospital in Malawi's Central Region that provides hysterectomies. Unfortunately, the wait time can be very, very long. Women may not be treated in time or may stop showing up for scheduled surgeries as they continue to be delayed. Other private facilities provide hysterectomies, but at rates that are unaffordable to our patients.

What are the alternatives to this treatment?

For patients with cervical cancer, there are no good alternatives. For patients with fibroids, it is possible to remove the fibroid alone. This procedure is often done on young women who are still of childbearing age. Some women with fibroids or bleeding receive hormonal treatment before attempting a hysterectomy. In many cases, less aggressive treatments are tried before the patient receives a hysterectomy.

Magdalena is a two-year-old toddler from Tanzania. Her family depends on crop cultivation for both sustenance and income. Three of Magdalena’s siblings are school-going. In 2022, tragedy struck as Magdalena sustained severe burns on her hip, extending down to her thigh, as a result of an accident with a hot stove while she was playing. Although she required medical attention for the extensive burn wounds, her family's financial circumstances prevented them from affording extended hospital care for her. Consequently, Magdalena received only initial first aid and continued her treatment at home. Unfortunately, as her wound healed, her thigh became fused to her waist, impairing her ability to use her right leg. Since then, Magdalena’s family has been struggling to find proper treatment for her condition. During one of our medical partner care center's outreach visits, the team met Magdalena and her family and recognized the severity of her situation. The family was directed to Kafika House, where a plastic surgery camp was being held. Despite the arduous journey of over nine hours, they arrived seeking hope and help. Upon examination, Magdalena was diagnosed by a plastic surgeon who determined that she would require a release procedure to separate her thigh from her waist. However, due to the complexity of her treatment, the surgeon has clearly communicated the possibility of amputating her leg if complications arise during the surgery. Magdalena is scheduled for surgery March 7th and her family is requesting help in raising $1,088 for surgery and postoperative care. Magdalena’s mother says: “I hope my daughter’s treatment will go well and that she will be able to walk after she completes her treatment.”

Cristina lives with her mother, a food vendor; her father, a construction worker; and eight older brothers and sisters in a neighborhood of La Paz, Bolivia. As a small baby, she struggled to breathe normally, and so her mother took her to the children's hospital where she was diagnosed with ventricular septal defect, a hole between the two lower chambers of the heart. Cristina also has Down syndrome. During surgery, doctors will sew a patch over the hole so that blood can no longer leak through it. Christina needs your help to raise $1,500 to pay for her heart surgery. Her mother says, "Our family is so grateful to know that there is a way to fix our daughter's heart so that she can breathe normally."

Diana and her husband Isaya just had beautiful newborn twins! Their family lives in the Arusha region of Tanzania. Diana works in the home, managing the household and caring for their children, while Isaya works diligently on their farm to provide food. In addition to farming, Isaya seeks daily work to supplement their income. Since the babies have been born the family has been working hard to try to provide them with everything they need. Diana's newborn babies were born with clubfoot. Clubfoot is a condition in which the foot is twisted out of shape. This will cause difficulty walking and even wearing shoes throughout his life. Fortunately, their family traveled to visit our medical partner's care center, Kafika House. There, the team will begin clubfoot treatment on February 20th. Our medical partner, African Mission Healthcare, is requesting $935 to fund their baby's clubfoot repair surgery. After treatment, his feet will grow straight to be able to walk easily and wear shoes. Diana Says, “I hope both my children will grow up with no disability.”
