Shoni is a 15-month-old baby boy from Uganda. According to his mother, Faidah, he has not been eating properly for the last month. He was taken to the local health center, where he was diagnosed with severe malnutrition.

Malnutrition is very treatable. Therapeutic milk products and micronutrients will help Shoni recover. Treatment will help him gain weight and strengthen his immune system. Shoni is scheduled to begin treatment on January 15.

Shoni is a very active boy who enjoys music, dancing, and playing with others. He is just beginning to talk. He is the only child of Joshua, a farmer, and Faidah, a tailor. They cannot afford the hospital and treatment fees. Our medical partner, The Kellermann Foundation, is requesting $316 to fund this treatment.

What kinds of symptoms do patients experience before receiving treatment?

At our medical partner's care center, Bwindi Community Hospital, two types of malnutrition are treated on an in-patient basis: moderate acute malnutrition and severe acute malnutrition. Moderate acute malnutrition (MAM) is also called “wasting” and is characterized by low weight-for-height indicators or by low mid-upper arm circumference (MUAC) indicators. Severe acute malnutrition (SAM) is the most dangerous type of malnutrition. It is caused by extreme deprivation of vital nutrients and becomes life-threatening because of its alterations of important functions of the body. SAM can manifest in two ways: severe wasting and oedema. Severe wasting is caused by extreme nutrient and calorie deficiency. Its symptoms include a massive loss of body fat and muscle tissue. This results in “baggy pants syndrome," in which the skin is loose while the body is extremely thin. Malnutrition of this type is also called marasmus. Oedema is caused primarily by the deficiency of protein in the diet. The body's extremities become extremely swollen. The oedema then progresses to the face and other areas of the body. Other symptoms include skin lesions, an enlarged liver, and changes in hair color. Malnutrition of this type is also called kwashiorkor. Kwashiorkor is the most common type of malnutrition treated at Bwindi Community Hospital.

What is the impact on patients’ lives of living with these conditions?

If not treated, moderate acute malnutrition can quickly progress to severe acute malnutrition. Chronic malnutrition can cause long-term growth and development issues, such as stunting and reduced cognitive capacity. Untreated, severe acute malnutrition can result in death.

What cultural or regional factors affect the treatment of these conditions?

There are many underlying causes of acute malnutrition in sub-Saharan Africa, including poverty, family size, lack of nutritional knowledge, mental health issues in caregivers, disease, war, social problems, and lack of clean water. Treatment is necessary to prevent malnutrition from becoming chronic and having a long-term impact on a child's development.

What does the treatment process look like?

Bwindi Community Hospital has a robust program for the treatment and prevention of malnutrition. Combining multifaceted community education, the assessment of nutritional status of children, and treatment of acute malnutrition, its goal is to prevent all forms of malnutrition. Hospital in-patient treatment, supported by Watsi, is reserved for the most acute cases. Every three months, the hospital’s Community Health Nursing Team (CHT) works with Village Health Teams (VHTs) to assess the nutritional status of all of the approximate 10,000 under-five children in its catchment area. Milder cases of malnutrition, which are the majority, are referred to district health centers for management. Early case-finding and treatment prevents progression to life-threatening, expensive, and complicated malnutrition. In addition, the CHT and VHTs conduct health education classes for the community. Subjects covered include family planning, sanitation and hygiene, maternal health, and prevention of illness. All of these issues are related to malnutrition. Once admitted to the hospital, a child is given a series of milk formulas. These formulas are calibrated to carefully increase nutrient and protein intake. After the formula phase, the child transitions to “Ready to Use Therapeutic Food” (RUTF). At Bwindi Community Hospital, the RUTF is a peanut butter-based food called plumpyNut™. It is nutrient-rich and packed with a high concentration of protein and energy. Supplements, such as Vitamin A and folic acid, are given. Antibiotics are given, if needed, to treat concurrent infections. After transitioning to the RUTF, the child is given an appetite test. If he or she eats well, the child is discharged and returns home with a supply of plumpyNut™ to supplement local foods. While the child is in the hospital, his or her caregiver receives health and nutritional education, including cooking classes, to help prevent recurrence of malnutrition. Food from a demonstration nutritional garden is used in the cooking classes and provided free to patients. When discharged, the child is referred to a local health facility and community nurse for follow-up. The child continues receiving treatment and supplemental food until his or her goal weight is reached.

What is the impact of this treatment on the patient’s life?

If the correct treatment is started promptly, a patient’s life can be saved. Any long-term impacts, such as stunting or cognitive development issues, can be mitigated or prevented. The child’s development is put back on track.

What potential side effects or risks come with this treatment?

There are no side effects or risks with this treatment.

How accessible is treatment in the area? What is the typical journey like for a patient to receive care?

Care for malnutrition without severe complications is available in district health centers, which is where most children are treated. When complications arise, adequate treatment is only available in hospitals. Patients are usually referred to the hospital by a community health team. They generally travel from 20 to 50 kilometers away and arrive by either walking or traveling on a hired motorcycle.

What are the alternatives to this treatment?

There are no alternative medications to treat acute, complicated malnutrition. Alternative hospitals are more than two-hour drive away.

Rama is a 27-year-old day laborer from Cambodia. He is married and lives in Ratanakiri Province with his wife, who is also a day laborer. They have two children; a son in Grade 1 and a three-year-old child. In his free time, Rama enjoys caring for his children and growing vegetables around his home. Two years ago, Rama was involved in a motorcycle accident in Ratanakiri, which caused a fracture of his thighbone. Following the accident, he underwent surgery at a local hospital, where doctors fixed the fracture using a metal plate and screws. A year later, he developed a fistula and had one screw removed at Children's Surgical Centre. However, the remaining hardware has brought on persistent discharging sinus, along with pain and difficulty in walking. The symptoms have also made it difficult for him to continue working as a day laborer, impacting his ability to provide for his family. Fortunately, surgeons at our medical partner, Children's Surgical Centre, can help. On July 1st, Rama will undergo a hardware removal procedure, which will help the wound heal properly, relieve pain, and improve his ability to walk. This will allow him to return to work and care for his family more comfortably. Rama and his family are able to contribute towards the cost of his treatment, but need help raising $303 more to fund the procedure. He said, “I am grateful to have this opportunity for treatment. I hope I can recover well, return to work, and spend time with my children without pain.”

Robinas is an intelligent 11-year-old boy who lives in Ethiopia with his parents and cousin. His father is a teacher who works hard to provide for the family, while his mother has been living with a chronic illness that has kept her at home for several years. Despite these challenges, his parents have remained committed to supporting both his education and his health. Robinas is on a full scholarship at a private school, where he consistently ranks among the top students in his class. His dedication and hard work have earned him several medals in recognition of his outstanding academic performance. Outside the classroom, he enjoys playing football, reading books, and spending time with his cousin. Robinas was born with hypospadias, a condition that causes urinary dysfunction. Although his parents noticed the condition at birth, they were advised to delay surgery until he was older, as the procedure would be safer once he had developed further. Over the years, the family visited several hospitals but were consistently told to wait. Eventually, doctors at a private hospital confirmed that Robinas was eligible for surgery; however, the cost was beyond the family’s financial means. They were then referred to a public hospital, where they faced a long waiting list. This prolonged waiting period has been emotionally difficult for the family, especially as Robinas has begun asking his father why he is different from other boys. After six months, the family finally received wonderful news. They were informed that Robinas had been referred for surgery at our partner’s medical care center, BethanyKids Myungsung Christian Medical Centre (BKMCM). Robinas is scheduled to undergo corrective surgery on June 29th. Our medical partner, African Mission Healthcare, is requesting $1,293 to cover the cost of his procedure and care. Robinas’s father shared, “Thank you so much for BethanyKids’s help with the children and for supporting my son. I feel terrible when I’m unable to afford that surgery, but the support means so much to us. Thank you again, God bless you all.”

Hadassa is a seven-year-old girl from Haiti. She is the middle child in her family, with an older sister and a younger brother. She does not currently attend school but enjoys helping her parents around the house and watching cartoons. Hadassa has a cardiac condition called a partial atrioventricular canal defect. A hole exists between the chambers of her heart, causing blood to leak between them instead of properly circulating through the lungs to receive oxygen. This leaves her weak and short of breath. Hadassa also has Down syndrome. Hadassa needs to fly to the Dominican Republic to receive treatment. On June 24th, she will undergo heart surgery, during which doctors will repair the hole in her heart. Another organization is paying for the surgery, but her family also needs help with travel and surgery prep. Our medical partner, International Cardiac Alliance, is requesting $1,500 to cover labs, medicines, and check-up and follow-up appointments. It also supports securing travel documents for the family and travel costs for the social workers from our medical partner, who will accompany Hadassa's family overseas. Hadessa’s mother shares, “Our family is so happy to know that there is a way to fix our daughter’s heart, and we hope everything will go well!”
