Siengly is 23 years old. He has six siblings. He likes to stay at home and watch TV. He is learning to repair smart phones in his free time.

Five days ago, Siengly developed chronic pain in his left groin and hip. It has become difficult for Siengly to walk. He went to a hospital in Phnom Penh for treatment, but his symptoms did not improve.

Siengly heard about our medical partner, Children’s Surgical Centre (CSC), from his neighbor. He traveled for two hours with his mother to reach CSC for treatment.

When he arrived, surgeons at CSC informed him that he will need an arthroplasty to repair his hip joint. The arthroplasty will help restore the function of his joint. His surgery is scheduled for April 6. The treatment will cost $450, and Siengly needs help to raise this money.

After his treatment, he will be able to walk more easily and will be able to live comfortably again.

Siengly underwent hip surgery.

Siengly’s treatment for his hip went well. Surgeons at CSC performed a debridement procedure to remove damaged and infected tissue, and the bone was additionally biopsied. Siengly returned to CSC for a follow-up appointment a few weeks later. His wounds have healed nicely, and he reports that he can walk much more comfortably and easily than before his procedure. Siengly is scheduled for another follow-up appointment next month, however he can return to CSC for free treatment if he experiences any problems before then.

Siengly says, “I’m so happy with the results of the surgery. I feel better and I don’t feel pain. I can do anything by myself. Thanks to the staff for helping me.”

What kinds of symptoms do patients experience before receiving treatment?

An arthroplasty is performed when a joint is destroyed, causing severe pain and difficulty walking. Joints may become damaged through fracture, trauma, degenerative joint disease, or congenital hip dysplasia. Other possible reasons include conditions developed during growth, such as Legg-Calve-Perthes disease, slipped capital femoral epiphysis, and avascular necrosis.

What is the impact on patients’ lives of living with these conditions?

When patients have destroyed joints, they experience pain and are immobile. This typically prevents them from working and supporting their families.

What cultural or regional factors affect the treatment of these conditions?

Traffic accidents are a leading cause of disability and death in Cambodia. Cambodian roads are in poor condition, and drivers are rarely safe. Fractures, dislocations, and trauma from traffic accidents are not uncommon in Cambodia.

What does the treatment process look like?

In order to replace a destroyed joint, surgeons must completely remove it. An incision is made over the affected joint, and dissection is carefully performed down to the bone. A saw is then used to carefully cut the ends of the joint away. The deformed, arthritic bone is removed. In some cases, an artificial metal prosthesis is then fitted to the ends of the bone and secured. A plastic bearing is then placed between the two metal ends of the joint so that the joint can move with low friction. The wound is closed. The patient will typically receive physical therapy, teaching him or her range of motion exercises and strengthening techniques.

What is the impact of this treatment on the patient’s life?

The patient is able to walk without pain and can return to work to support his or her family.

What potential side effects or risks come with this treatment?

Surgery always carries a risk of a death, but such a complication is very unlikely. This surgery is highly successful.

How accessible is treatment in the area? What is the typical journey like for a patient to receive care?

Surgical treatment is not easily accessible in rural Cambodia. In cities, it is too expensive for many patients to afford. Patients travel anywhere from 30 minutes to eight hours to reach Children's Surgical Centre (CSC) for treatment. They learn about CSC from family members or neighbors who have received treatment there. They travel with their family members via motorcycle or taxi.

What are the alternatives to this treatment?

Patients seek Khmer traditional medicine. They visit local healers, who provide topical and consumable treatment that is often not effective.

Telake is an adorable 4-year-old girl who loves to play with her friends. She loves to color, write, and play with her baby sister. Telake's mother stays at home while her father is a tuk-tuk driver. A tuk-tuk is a three-wheeled mother vehicle used as a taxi. At times when he doesn’t get a job, he does casual labor work. Telake's mother noticed swelling on the left side of her child’s stomach. Doctors at BethanyKids Myungsung Christian Medical Centre (BKMCM), diagnosed her with inguinal hernia, a birth condition where there is a weakness in the abdominal wall. In infants and children, inguinal hernias are operated on to prevent other complications. Symptoms may include pain, a burning sensation, swelling, and discomfort. Fortunately, on March 19th, she will undergo hernia repair surgery at our medical partner's care center, BKMCM. Our medical partner, African Mission Healthcare, is requesting $487 to fund Telake's surgery. Once completed, this procedure will hopefully allow her to live more comfortably and confidently. Her mom said, “I hope she will heal after the surgery. My daughter wants to buy me a house when she grows up. I wish she grows up independent and educated."

Siltan is a 4-year-old girl from Ethiopia. She loves to play with her mom and her dolls. Her parents are farmers and the income from their harvest is used to sustain the basic needs of the family. Siltan was born with an anorectal malformation, a condition where there is a complete or partial intestinal blockage causing bowel obstruction. Siltan is scheduled to undergo corrective surgery on March 12th. Our medical partner, African Mission Healthcare Foundation, is requesting $1,500 to cover the total cost of Siltan's procedure and care. After her recovery, Siltan will no longer experience bowel dysfunction or be at risk of developing health complications in the future. Siltan's dad said, “I have hopes that she will heal. I will educate her and help her to be a doctor.”

Phyu is a 32-year-old mother of three from Thailand. She resides with her family in an Internally Displaced Persons (IDP) camp along the Thai-Burma border where she also teaches at the camp school. During her free time, Phyu enjoys spending quality time with her family. Since August 2023, Phyu has been experiencing pain in her abdomen, pelvis, and lower back, along with irregular menstruation. She has been diagnosed with left ovarian cyst, and has been advised to undergo a hysterectomy, which surgically removes of her uterus and ovaries. If left untreated, Phyu's symptoms will continue to worsen and put her at risk for further health complications in the future. Fortunately, Phyu is scheduled to undergo her hysterectomy on February 23rd. Our medical partner, Burma Children Medical Fund, is requesting $1,500 to cover the total cost of her procedure and care. Once recovered, Phyu will be relieved of her pain and able to resume her daily activities. Phyu said, “With my current condition, I cannot teach my students, do household chores for my children, or assist my husband. I am looking forward to a complete recovery soon."
