Sori Yanny is sixteen years old and in the eleventh grade. He has three sisters and one brother. He likes to play badminton and soccer and to listen to music.

In June, Sori Yanny was in a motorbike accident, which caused a fracture in his right humerus and forearm. He visited a Khmer traditional healer for treatment, but his symptoms did not improve. Sori Yanny could not extend his wrist, making it difficult for him to hold objects.

When Sori Yanny learned about our medical partner, Children’s Surgical Centre (CSC), he traveled for four hours to seek treatment. On November 23, CSC surgeons performed a tendon transfer procedure. After recovery, Sory Yanny will be able to use his right arm again. Now, his family needs help to fund this $450 procedure.

Sori Yanny underwent successful fracture repair.

Surgeons performed a nerve and tendon transfer in his right arm. Following his procedure, he was instructed to use a splint for six weeks. He will also have six weeks of physiotherapy. Sori Yanny is able to use his arm better, and he does not experience any pain.

A number of nerve and tendon procedures are performed at Children's Surgical Centre. These are typically performed because a nerve is no longer fully functional. Reasons for this limited functionality can include lacerations, blunt trauma, burns, and tumors. Sometimes, spinal nerves are compressed due to herniated discs, bone spurs, and tumors.

What is the impact on patients’ lives of living with these conditions?

The patient cannot work or drive a motorbike, the most common form of transportation in Cambodia.

What cultural or regional factors affect the treatment of these conditions?

Traffic accidents, common in Cambodia, are the most typical cause of a brachial plexus injury (BPI), or damage to a network of nerves on the neck and shoulders. The inability to use a limb is debilitating for patients, whose livelihoods involve physical activity. They are often farmers, factory workers, or drivers.

What does the treatment process look like?

When a nerve no longer functions, the muscle that it innervates no longer functions. Surgeons either repair that nerve or use a nerve from somewhere else in the body to act as a graft. In some cases, a nerve can be redirected from a less important muscle and grafted into a more critical nerve. During a BPI surgery, surgeons divert a nerve from one destination and sew it into the non-functioning nerve. It can take three to six months before a patient fully recovers from this operation.

The patient will regain function in his or her arm. The patient can drive a motorbike and go back to work.

What potential side effects or risks come with this treatment?

This surgery is highly effective. The nerve is already damaged, so there is no risk of causing more harm. The surgery can only improve function.

How accessible is treatment in the area? What is the typical journey like for a patient to receive care?

This surgery is not available to most Cambodians. Patients travel up to twelve hours to visit Children's Surgical Centre (CSC). They learn about CSC from the radio, a neighbor, or a family member.

What are the alternatives to this treatment?

There are no alternative treatments to regain limb functionality.

Christine is a small-scale farmer from Uganda. She is married and blessed with four children aged between 17 years and 1 year who are all in school except the last one. Her husband is a builder, and they live with her parents in a two-room mud house for shelter. They combine efforts to develop and support their family. Eleven years ago, Christine began to experience troubling symptoms, including a neck swelling associated with pain, difficulty in swallowing, and loss of voice. She is also unable to carry heavy items and experiences headaches. She visited Rushoroza Hospital for treatment. She was diagnosed with a goiter and surgery was recommended to prevent her symptoms from getting worse. Our medical partner, African Mission Healthcare, is helping Christine receive treatment. She is scheduled to undergo a thyroidectomy on February 24th at our medical partner's care center. Surgeons will remove all or part of her thyroid gland. This procedure will cost $333, and she and her family need help raising money. Christine says, ‘‘I believe and pray to have a successful surgery. I hope to continue with farming after my recovery to be able to support my husband and our family. May God bless you.’’

Mariam is a newborn baby in a family of two. Mariam's parents are farmers who work diligently on their farm to provide food and sell surplus harvests for additional income. Miriam was born with clubfoot on both feet. Her feet are twisted inward and downward. This causes difficulty walking and even wearing shoes. Fortunately, Mariam's family traveled to visit our medical partner's care center, Kafika House. There, the team will begin clubfoot treatment on April 5th. Our medical partner, African Mission Healthcare, is requesting $935 to fund Mariam's clubfoot repair. After treatment, her mobility will improve, and she will be able to wear shoes properly. Mariam’s mother says, “Please help my daughter. I hope she gets treatment and grows up to enjoy a life free from disability.”

Saream lives in Cambodia with her niece's family. Her niece and husband work as grocery sellers in the village, while Saream stays home as a caretaker. In her free time, she likes listening to Cambodian traditional music and cooking. Saream and her niece traveled two hours to reach the Children's Surgical Centre for consultation regarding scar tissue contractures on her ankle. Due to the swelling and restriction of movement, Saream finds it difficult to walk and get housework done. On March 4th, Saream will have a contracture release surgery. This surgery will improve her mobility and ability to carry out everyday tasks at home. Now, the family needs help to fund the $487 procedure, including post-op care and physiotherapy. Saream shared, "I hope I can fix this problem and be able to take care of my family."
