Srey Na underwent nose reconstruction surgery.

Srey Na and her family traveled for one and a half hours for consultation and surgery. She remained at the hospital for several days after surgery to make sure she was healing well and was given antibiotics to clear up any infections. Srey Na will rest at home for a week: most patients experience full recovery within about 4-6 weeks. Srey Na looks forward to helping her husband at the market and feeling well enough to care for her family.

Srey Na is a 29-year-old woman from Cambodia. She is married and sells fish at the local market with her husband for a living. She and her husband have one son, who is seven years old and in first grade. Srey Na stays busy caring for her son, doing housework, and sewing clothes for her family.

For the past two years, Srey Na has experienced ear pain and discharge, frequently accompanied by headaches and fevers. Recently, her symptoms have worsened, and she has experienced ringing in her ears and chronic headaches. She has trouble sleeping and her hearing has diminished in both ears. Srey Na has been diagnosed with chronic rhino-sinusitis and serous otitis media in both ears. This happens when tissue that lines the sinuses begins to swell, trapping mucus that typically flows through the sinuses and out through the nose. The trapped fluid can grow bacteria that can cause infections.

Fortunately, our medical partner, Children’s Surgical Centre, is helping Srey Na receive treatment. On December 15th, she will undergo a functional endoscopic sinus surgery (FESS). Now, Srey Na needs help raising $333 to fund her procedure and care.

The patient experiences pain, bleeding, nasal congestion, and sometimes swelling of the nasal septum.

Patients experience difficulty breathing and sleeping due to nasal congestion, as well as a loss of confidence because of the disfiguring of their nose.

What cultural or regional factors affect the treatment of these conditions?

Motorcycle accidents are the leading cause of death in Cambodia, and as such, most fractures are caused by falling or being struck as a result of the accident.

What does the treatment process look like?

The patient first consults with our staff and a treatment plan is arranged as follows: Day 1 - the patient takes an x-ray to confirm the fracture. Then, if the accident was caused less than a week prior, the patient will undergo antibiotics for one week before the operation is scheduled. Operation day - the procedure can last anywhere from 30 minutes to one hour and the patient will then recover at CSC for one night. Day After Operation - the patient returns home operation. Day 5 After Operation – Patient returns for removal of bandages and nasal packing (to keep the bones in place). One month after operation – Follow-up appointment is scheduled.

What is the impact of this treatment on the patient’s life?

The patient will be able to breathe normally, sleep without disturbance, and the bones will heal back into alignment.

What potential side effects or risks come with this treatment?

Success rates are very high, however, with all surgeries, some patients do not respond well to treatment and in a small number of cases, the outcome can pose no improvements.

How accessible is treatment in the area? What is the typical journey like for a patient to receive care?

Nasal reconstruction operations are accessible at a limited number of public and private clinics and hospitals at a cost that a majority of Cambodians cannot afford. Patients travel up to twelve hours to reach the Children's Surgical Centre for surgery, often taking multiple forms of transportation and finally arriving by bus, motorbike, or taxi.

What are the alternatives to this treatment?

In some cases of infection, the condition can be treated by an emergency incision near the swelling of the nasal septum. However, it can sometimes provide only partial relief. Alternatively, Cambodians in rural areas may seek out less expensive options, such as traditional Khmer healers, to relive their symptoms. But this type of medicine does not yield sustainable or cost-effective results.
