Mathew underwent hypospadias repair surgery to allow him to live with comfort.

Mathew underwent surgery, which was successful! He is currently scheduled for a clinic review with the possibility of getting a second-stage hypospadias repair in six months if required. Once recovered, Mathew will be free from related health issues, psychological impact, and future complication risks.

Mathew’s brother—who was helping care for him—told us, “I am happy my brother has received the surgery. I know this will boost his self-esteem and allow him to grow like other boys.”

Mathew is a teenager from Kenya. Currently, he is not in school and herds the family cattle as part of his daily activities. He is the fourth-born in a family of nine children, and his first-born brother is the breadwinner. He does bodaboda business (motorcycle taxi) and has a pool table in the community where he charges a few shillings per game.

Mathew was born at home, and a few days later his older brother noticed that he was not passing urine properly. His parents didn’t take any action, thinking it was normal. His brother didn’t know what to do at the time because he was not old enough to take Mathew to the hospital.

Now 17 years later, Mathew’s brother decided to take him under his wing and take him to the hospital despite not being subscribed to any health insurance program. They visited several hospitals, but nothing was done. A neighbor then referred them to a nearby clinic.

Mathew was examined at the clinic and referred to BethanyKids for further review. His brother gathered some funds and managed to raise enough to bring Mathew for examination.

Mathew was born with hypospadias, a congenital abnormality that causes urinary dysfunction. Without treatment, he will continue to experience uncomfortable symptoms and will be at risk of infertility and cancer.

Fortunately, Mathew is scheduled to undergo corrective surgery on December 14th. Our medical partner, African Mission Healthcare Foundation, is requesting $847 to cover the total cost of his procedure and care.

Hypospadias is a congenital defect in which the opening of the urethra is on the underside of the penis. The urethra is the tube that drains urine from the bladder. In males, the opening of the urethra is normally at the end of the penis. Symptoms of hypospadias vary. This condition may cause genital malformation and urinary dysfunction. It can lead to infections, social stigma, and infertility.

If this condition is not repaired, it can lead to urinary dysfunction, genital malformation, infertility, and increased risk of urinary tract infections.

What cultural or regional factors affect the treatment of these conditions?

Hypospadias is one of the most common birth defects in boys. It is the most frequent congenital urological anomaly, occurring in 1–3 per 1,000 live births.

What does the treatment process look like?

After surgery, the patient is monitored closely and discharged from the hospital after five days. Stitches will be removed during a follow-up appointment.

The urethra will be corrected, improving urinary function.

This is a low-risk procedure, and the impact of this surgery lasts a lifetime. If the patient has complicated hypospadias, he may need to undergo further surgery. Follow-up visits with a urologist may also be needed, particularly when patients reach puberty.

How accessible is treatment in the area? What is the typical journey like for a patient to receive care?

There are few quality care centers in the region. Hospitals lack adequate resources and expertise to treat this condition.

Surgery is not required for mild cases. Otherwise, there is no alternative.

Lwin

Lwin is a 37-year-old wife and mother who lives with her husband and son on the Thai-Burma border. Lwin and her husband work as day labourers in a temple as a cook and gardener. In her free time, Lwin likes to garden and enjoys growing vegetables to eat and to donate to the temple. Lwin also likes to watch Thai movies. One night in March 2021, Lwin had a rapid heartbeat and felt dizzy. Lwin could not sleep that night so her husband brought her to a clinic the next day. The doctor referred her to Mae Sot Hospital in an ambulance, where she underwent diagnostic tests, but never received a diagnosis. Lwin was given medication to stabilize her condition. During her follow-up appointment in June 2022, Lwin received an echocardiogram and was diagnosed with mitral valve stenosis. The doctor then referred her to Chiang Mai Hospital for further treatment. Currently, Lwin suffers from shortness of breath and heart palpitations. Lwin reports her arms and legs feeling heavy and trouble sleeping. Lwin has little appetite and she has to sleep propped up on pillows. On January 13th, Lwin with receive surgery to improve her condition. Our medical partners, Burma Children Medical Fund, are helping Lwin raise $1,500 to cover the cost of this life altering surgery. Lwin shared, "I will never forget the donors who will help pay for my treatment cost. When I recover fully, I want to learn how to sew so that I can open a shop from my home and work as a seamstress."
